The Home Care Liaison facilitates referrals to home care and/or hospice services by gathering and interpreting medical and financial information, meeting with patients and their families, and collaborating closely with case managers. This role ensures a smooth and effective transition from the hospital or other care facilities to the patient’s home environment.
Key Responsibilities:
- Conduct initial clinical, social, psychological, and financial assessments to determine eligibility for home care and/or hospice services.
- Communicate timely and accurately with the agency via phone, fax, or electronic systems to ensure complete documentation and data entry.
- Assist patients in verifying insurance coverage for home health and/or hospice services.
- Serve as a knowledgeable resource for case managers, physicians, hospital staff, and community partners by interpreting home care and hospice policies.
- Support public relations and outreach efforts to promote awareness and utilization of services.
Education & Experience Requirements:
- BSN preferred
- Active RN license and registration required
- Minimum of 2 years' experience in home health, hospice, or healthcare coordination preferred
