The Per Diem Stroke Neuro Nurse Practitioner performs the role utilizing the patient-centered coordinated care model demonstrated through the following competencies: leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision making. Works collaboratively with other members of the practice/hospital team to assess, plan and implement care for individuals with health and safety needs. Minimal 3-5 years demonstrated experience working with acute neuro/stroke population required. Stroke Certified Registered Nurse (SCRN) certification preferred.
