SUMMARY (Basic Purpose of the Job)
The PGY II Critical Care Pharmacy Resident collaborates with the multidisciplinary team to optimize patient outcomes in primarily the acute care and intensive care practice settings. They work to build upon the experience gained from their PGY I Pharmacy Residency to further promote the safe and effective use of drugs, dissemination of knowledge through educational and scholarly activities, and advancement of pharmacy practice. Additionally, the resident works in conjunction with the Pharmacy Management and Clinical Teams to develop clinical guidelines, oversee quality initiatives, and serve as a leader/preceptor for PGY I residents and pharmacy students. Lastly, they assume operational responsibilities as deemed appropriate by the Pharmacy Director and Residency Director. Graduates of the PGY II Critical Care Residency Program are expected to be well-prepared to pursue a position in Critical Care, Emergency Medicine, Internal Medicine, or Academia.
MINIMUM REQUIREMENTS
Education:
Graduate of an ACPE-accredited school of pharmacy. Doctor of Pharmacy (PharmD) or equivalent advanced degree. Eligible for licensure in New Jersey (attained within 60 days of start date or 11/1 per pharmacy administration discretion).
Experience:
Completion of PGY I Pharmacy Practice Residency required.
Other Credentials:
AHA ACLS-Adv Cardiac Life Supp(within 60 days of start date),AHA BLS - Healthcare Provider(within 60 days of start date),AHA PALS(within 60 days of start date)

ESSENTIAL FUNCTIONS
- Interprets, evaluates, and accurately processes drug orders for patients across a continuum of care. Efficiently and effectively utilizes pharmacy operational computer systems and integrated hospital systems. Follows policies and procedures to maintain accurate patient medication profile.
- Initiates appropriate medication therapy based on age, disease, and drug considerations for neonatal, pediatric, adolescent, adult, and geriatric patients. Actively intervenes to assure therapy is individualized based on medication factors and the patient’s physical and psychological needs.
- Identifies and resolves problems with medication orders or therapeutic regimens. Clarifies issues with prescribers and suggest order adjustments or alternative medication therapies when needed to prevent adverse events or adhere to institutional policies.
- Provides concise, applicable, comprehensive, and timely responses to requests for drug information from patients and healthcare providers.
- Supervises and coordinates pharmacist, residents, students, and technician work processes as appropriate for the management of inpatient and critical care practices.
- Ensures proper drug utilization by adherence to formulary criteria and pharmacy policies. Develops, leads, and supports clinical pathways and quality assurance programs.
- Communicates between shifts and with peers to ensure continuity of care and optimal delivery of pharmaceutical services.
- Participates in management of medical emergencies and respond to adult/pediatric codes. Actively leads medication management processes as a member of the Code Blue team.
- Demonstrates ownership of and responsibility for the welfare of the patient by performing all necessary aspects of the medication-use system. Displays initiative in ensuring that patients receive comprehensive and individualized pharmaceutical care. Provides oversight for clinical services in a variety of patient populations. Coordinates all clinical and direct patient care pharmacy services for various specialties. Responds to consultation for pharmacy interventions and makes appropriate evidence-based, patient-centered medication therapy recommendations.
- Works as an active member of multidisciplinary team and collaborate with healthcare providers within patient care area. Attends multidisciplinary work/care rounds with specialty service(s)/clinics as appropriate. Ensures that accurate and timely medication-specific information regarding a specific patient is communicated to those who need it at the appropriate time. Provides written documentation in the patient’s medical record when requested or directed by policy.
- Documents pharmaceutical care interventions in departmental reporting system.
- Actively identifies, mitigates, prevents, and reports potential and actual medication errors or adverse drug events. Recommends rational changes to medication systems as needed to prevent future problems. Implements, evaluates, and redesigns integrated evidence-based pharmaceutical care plans based on patient progress and evolving goals.
- Establishes collaborative professional pharmacist-patient relationships. Educates patients regarding safe and effective use of medications, possible medication side effects, and the role of prescribed medications in the overall therapeutic care plan. Provides medication history/reconciliation, discharge counseling, outpatient medication review and education as appropriate to facilitate the transition of care. Identifies opportunities for improvement of the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs. Participates in planning, implementing and evaluating new and existing programs.
- Maintains and enhances up-to-date evidence-based knowledge of drug and drug therapy by reviewing relevant professional journals and suggested readings.
- Demonstrates teamwork within the Pharmacy Department and Capital Health Critical Care Services. Participates in the group approach to patient care by providing support to peers and other practice areas as needed. Serves as an internal and external liaison for the goals and vision of the Pharmacy Department and Capital Health Critical Care Services. Demonstrates dedication and loyalty to the department and colleagues. Represents Pharmacy Department at hospital meetings and events as needed. Participates actively with the Pharmacy & Therapeutics Committee and other committees/taskforces as requested. Serves as a preceptor-in-training/primary preceptor for programs PGY-I residents and student experiential education.
- Designs and implements quality improvement changes to the organization’s medication-use system. Conducts practice-related projects, clinical research, or pharmacy CQI initiatives using effective project management skills. Reviews protocols, orders, and policies as requested.
- Provides effective evidence-based medication and practice-related education to healthcare professionals and members of the department. Reviews, evaluates, and discusses the results of investigations of pharmacy practice-related issues. Provides updates of therapeutics, policies and procedures, formulary, or regulatory issues as needed. Participates in the creation and dissemination of pharmacy newsletters for nursing, medical, and ancillary staff.
- Participates in the orientation, education, and mentoring of new staff (i.e. pharmacists, pharmacy residents, nurses, pharmacy technicians, etc.) in addition to community outreach initiatives.
- Participates in activities related to interpersonal leadership and development of management skills. Maintains a sustained record of contribution to the total body of knowledge in pharmacy practice through presentations, publications, and professional affiliation activities. Maintains a professional profile and active membership in relevant pharmacy/medical organizations. Attains membership in other local, state, or national professional organizations and is highly encouraged.
- Completes core rotational and longitudinal experiences as scheduled to achieve mastery of the required and specific elective goals and objectives defined by the pharmacy residency advisory committee and ASHP Accreditation Standards.
