Provides direct patient care including evaluation and treatment as outlined by the guidelines of the New York State Physical Therapy Practice Act and Hospital Policy. Provides daily clinical supervision to P.T.A.'s, Rehabilitation Aides and students. Embraces the team concept of the department and the customer service approach to patient care.
Responsibilities
- Provides direct patient care including evaluation, treatment, equipment assessment and documentation.
- All patients are evaluated within 24 working hours of receipt of prescription, each patient contact is documented in the medical record and equipment is provided or ordered as appropriate 100% of the time.
- Maintains own patient schedules and utilizes time efficiently.
- Therapists maintains a full patient case load as per service requirements and Departmental policy.
- Maintains accurate patient records, statistical reports and other documentation related to patient care.
- Patient records and statistical data are completed daily 100% of the time.
Submits statistical composite on a monthly basis.
- Performs patient and Family education and attends Family meetings and educational sessions.
- All patient and families, when available receive education 100%of the time as evident io daily documentation.
- Participates in departmental performance improvement activities, program planning and equipment needs.
- Therapist provides performance improvement data to supervisor on a monthly basis, discusses program development and recommends equipment for departmental use.
- Attends departmental educational activities including in-services and continuing education courses and Interdisciplinary Conferences.
Provides formal in-service presentations to physical therapist start and various hospital personnel
- Attends at least one or two outside courses/conferences annually and provides in-services in all completed conferences and attends 100% of scheduled in-services and Interdisciplinary Conference meetings.
- Supervises and delegates duties to supportive personnel including P.T.A.'s, Rehabilitation Aides. and volunteers.
- Therapist delegates duties appropriately and supportive personnel verbalizes comprehension of instruction.
- Supervises physical therapy students and serves as clinical Instructor following 1 year of clinical experience.
- Supervises a minimum of 1 student annually and provides daily guidance and completes appropriate evaluations.
- Demonstrates awareness of Infection Control Policies and safety procedures in both patient care and physical environment.
- Therapist immediately notifies the supervisor of equipment malfunctioning and environmental problems.
Follows Infection Control policies 100% of the time.
- Maintains professional behavior in all patient and staff interactions.
- Therapist exhibits the ability to interact with all staff and patients.
- Core team members will actively participate in all aspects of the interdisciplinary care model.
- Therapists will attend daily meetings to communicate with team members regarding rehabilitation assessment goals, progress, and outcomes.
- Non-core team members will participate in interdisciplinary care model activities as necessary.
- Therapist will attend daily meetings as necessary as stated above.
Requirements
- BS accredited Physical Therapy program Required.
- MS accredited Physical Therapy program Preferred.
- 1-3 years' Inpatient experience Required.
- Experience in outpatient and acute care bedside services, including: MICU, CCU, SICU, CSICU, NICU.
- Orthopedics, vascular, neurology, neuroscience, oncology, medicine, cardiothoracic, lung transplant, other transplants, and pediatrics. Preferred.
- General therapeutic equipment.
- General computer literacy.
- Physical Therapists Required.
- OHS Fit Test Required.
- OHS Annual Assessment Required.
- New York State license and current registration or a limited permit for a recent graduate.
