Responsibilities:
The Hospital at Home (H@H) Resource Nurse competencies and accountabilities are consistent with the Clinical Level II Nurse.
The H@H Resource Nurse is an RN that practices as a full partner on the health care team and consistently demonstrates safe practice within the organizational professional practice model.
Additionally, the H@H Resource Nurse is a flexible staffing resource that must be able to visit patients in various geographic areas as determined by agency contracts including but not limited to, Hospital at Home, Penn Home Infusion, and Home Health when needed. Initial assignments will be in geographic areas roughly 30 minutes of travel distance from Penn Medicine Hospitals.
Hospital @ Home Essential Functions Utilizes monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home.
Ensures all interventions are implemented efficiently with a patient-centered approach. Leverages the resources and expertise of the interdisciplinary care team and our supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation.
H@H In-Home Nursing: Conducts home visits per schedule defined by nursing care plan (at lease 2/24 hours per patient) and in response to urgent need (within 30 minutes).
Maintains contact with the Command Center during a visit, participates in video visits, and confirms plan with the Command Center. Conducts initial and ongoing home and patient safety assessments, provides intervention should the home environment change or pose a safety risk for patients or staff.
Documents all assessment findings, interventions and response to interventions in the inpatient EMR (Epic- Penn Chart).
Resource Nursing: Conducts home visits for Penn Home Infusion Team and Penn Medicine Home Health when operationally appropriate to fill in for vacancies or other operational needs.
Documents all assessment findings, interventions, and response to interventions in outpatient EMR (Epic- Remote Client) Virtual Nursing (When applicable): There may be times when Resource RN may flex into the Virtual Nurse Role. The Virtual Nurse assesses patient and develops/maintains the nursing plan of care in alignment with the medical plan of care.
Monitors patients using remote monitoring tools and dashboard.
Responds to alerts, communicates directly with patients to resolve alerts.
Provides assessment, intervention and evaluation per nursing care plan carried out through virtual visits using video or audio communication.
Ensure patient communication regarding daily schedule and plan of care.
Documents all assessment findings, interventions and response to interventions in the inpatient EMR (Epic- Penn Chart).
The Hospital @ Home Resource Nurse possesses the knowledge, skills and the attitude to effectively meet standards for competent professional nursing practice as specified in the Clinical Nurse 2 position description addendum.
The Hospital @ Home Resource Nurse exhibits UPHS organizational behavioral competencies which reflect its mission and are embedded in the professional practice competency domains.
The Hospital @ Home Resource Nurse independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patients. Assigned patients may range in age from infancy to the elderly.
The Hospital @ Home Resource Nurse demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork (Definitions listed below).
Hospital @ Home Resource Nurse works closely with the interprofessional health care team to facilitate the coordination of care across the continuum. The nurse focuses on expanding the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age-appropriate considerations of assigned patients.
The Hospital @ Home Resource Nurse seeks feedback in assuming leadership roles and provides feedback for improved clinical practice. The nurse demonstrates how nursing practice impacts the organizational vision, mission, and goals and the care delivery system. The nurse complies with all regulations and standards of regulatory and accrediting bodies. Maintains and practices in accordance with nursing guidelines per state and hospital regulations.
Participates in discharge planning in order to provide continuity of care. Utilizes nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problems.
Exhibits sound clinical decision making and critical thinking skills.
Adheres to the clinical protocols/order sets and obtains the necessary training to provide care within the context of providing evidence-based care.
Promotes professional practice and a culture of safety. Engages in process improvement efforts. Serves as a role model for professional nursing practice.
Credentials:
- Registered Nurse - PA (Required)
- Basic Cardiac Life Support through the AHA (Required)
- Car Insurance (Required)
Education or Equivalent Experience
- Bachelors of Nursing OR Nursing Diploma or ASN from an accredited school of nursing
- More than 15 months relevant professional nursing experience and completion of the Nurse Residency Program
- ICU, ED, acute care experience
