The Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. The Hospital at Home Per Diem Nurse Practitioner will serve in a dual capacity. The APN will take a lead role in identification and assessment of potential Hospital at Home referrals, in collaboration with the Virtual Hospitalist. The APN must be committed to providing high-quality, evidence based, patient centered care to a diverse patient population in a non-traditional setting. The Hospital at Home Advanced Practice Nurse Provider (APN) will serve in a dual capacity. The APN will take a lead role in identification and assessment of potential Hospital at Home referrals, in collaboration with the Virtual Hospitalist.
