Job Description:
The Cardiology Valve Coordinator is a specialized healthcare professional responsible for coordinating all aspects of care for patients undergoing evaluation and treatment for valvular heart disease. This role serves as a critical liaison between patients, families, physicians, and the multidisciplinary team, ensuring a seamless continuum of care.
Shift Information
Essential Functions
- Serve as a coordinator for assigned program
- Discuss program with physicians, nurses and Cardiovascular leadership team and concurrently review that all care meets evidence-based guidelines
- Provide and/or oversee required education to patients and family members
- Collaborate with the director to oversee and ensure that the program meets the regulatory requirements
- Perform chart reviews and collect, analyze and report data for performance improvement activities, planning and evaluation of the program
- Maintain and/or analyze data collected on assigned program
- Promote program to internal and external audiences
Skills
- Manage patient referrals, evaluations, and follow-up appointments
- Serve as the primary point of contact for patients and families, providing education and guidance throughout the treatment process
- Coordinate diagnostic tests such as echocardiograms, CT scans, and catheterizations.
- Facilitate case presentations and discussions in multidisciplinary team meetings.
- Maintain and update patient records, ensuring compliance with regulatory and accreditation standards.
- Track program outcomes and provide reports to leadership.
- Work closely with surgeons, interventional cardiologists, and other healthcare providers to develop individualized treatment plans.
- Act as a resource for clinical staff regarding valve disease treatments and procedures.
- Monitor patient care quality and implement improvements as necessary.
- Support research initiatives and clinical trials related to valvular heart disease.
Minimum Qualifications
- Associate's degree or higher
- Registered Nurse (RN), Nurse Practitioner (NP), or Physician Assistant (PA)
- Current BLS certification endorsed by the American Heart Association
- Minimum of three (3) years of relevant clinical experience (i.e. Cath Lab Tech, RN, etc.),
- Strong organizational, communication, and interpersonal skills
Preferred Qualifications
- Experience in a coordination or case management role
- Bachelor's Degree in Nursing (BSN)
- Current ACLS certification
