Our Patient Education Specialist will possess a passion for Health Literacy and work with our patient education and patient experience team. We would like you to have an enthusiasm for teaching clinicians and how to best provide patient education, previous experience with project and program management along with a history of leading through influence. The Patient Education Specialist functions as a specialty care nurse who development implements, and evaluates education materials and resources for patients, families, and significant others. As a role model, change agent, and patient advocate, the Patient Education Specialists works collaboratively with interprofessional team members, patients, and families in assessing health knowledge and understanding of complex patient conditions. The PES is responsible for ensuring that patients with a variety of diagnoses received quality and comprehensive coordinated education related to identified patient care needs and anticipated follow-up care post hospitalization. The Patient Education Specialist promoted improved patient health outcomes through high quality education and materials on health, disease management, prevention, recovery, and patient/caregiver engagement in care. Maintains strategic partnerships for the development of patient education materials and advance the knowledge and skill in the delivery of patient education. Ensures compliance with standards of state, federal, regulatory, and accrediting agencies.
Requirements:
- 5 years recent clinical experience in inpatient/outpatient
- Current PA RN License
- MSN preferred
