Summary:
Coordinates care of assigned patients from admission to discharge and post-hospital care; assesses appropriateness of level of care, communicating with patients, families, medical staff and third party payers as necessary to ensure effective patient care and resource utilization.

License Requirement
License to practice as a Registered Nurse in the State of New York.
Education and Experience
1. Bachelor of Science Degree in Nursing and the equivalent of two (2) years of full –time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management; or
2. Associate’s degree in Nursing and the equivalent of four (4) years of full-time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management; or
3. Nursing diploma and the equivalent of five (5) years of full-time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management.
NOTE:
Required degrees must have been granted by an accredited college or university or one recognized by Roswell Park Comprehensive Cancer Center as following acceptable educational practices.
