Job Summary

This pivotal role is responsible for coordinating daily operations, scheduling clinical staff and patient visits, ensuring regulatory reporting, and serving as the clinical liaison across multidisciplinary teams. This roles helps maintain continuity of care and ensures high-quality service delivery that meets CMS and state guidelines for Hospital at Home care.
Essential Duties & Responsibilities
- Coordinates the day-to-day operations of the Hospital at Home program including staffing, patient assignments, and visit scheduling.
- Serves as the primary point of contact for clinical and operational communication between in-home staff, virtual providers, and hospital partners.
- Collects and submits CMS-required Hospital at Home data including patient demographics, care metrics, and program utilization reports.
- Interviews, hires, supports onboarding, scheduling, and supervision of Hospital at Home staff. Assists staff in their professional development and in the identification of needed resources to ensure optimal patient outcomes. Ensures staff is educated on risk management and compliance issues. Interprets and evaluates employee satisfaction survey data.
- Evaluates and manages staff competency to complete employee performance evaluation through direct observation, peer review, orientation and employee input.
- Collaborates with various departments to ensure patients have all necessary equipment and supplies
- Understands, articulates and ensures compliance with State Nurse Practice Act, State Board of Nursing Regulations, Regulatory agency standards and policies of the organization.
- Performs as Unit leader for quality improvement process. Recognizes trends in patient care delivery and explores opportunities for improvement which are evidence-based. Monitors patient care satisfaction and supports the Forces of Magnet.
- Utilizes evidenced based practice in the development of policies and procedures based on comparison data benchmarks. Designs safe clinical systems, processes, policies and procedures.
- Assists in preparing, planning, monitoring and executing budgetary standards, for both operational and Capital budgets. Educates team members on financial implications of patient care decisions.
- Integrates high ethical standards and core values into everyday work activities. Develops unit activities that support the elements of extending excellence in clinical practice. Advocates use of documented best practice.
- Consults and collaborates with the multidisciplinary team members to coordinate the management of patient care within the unit.
- Performs other duties as assigned.
Qualifications
MINIMUM REQUIREMENTS
Graduate of accredited school of nursing. BSN required.
Current NJ RN license in good standing. Must have applied for second out of state RN license (NJ or PA) or NJ Multi State license within three months from date of hire.
Three years progressive clinical and management practice in patient care area. Two years demonstrated competency in charge role.
AHA BLS - Healthcare Provider, Registered Nurse - NJ
Requires NIH Stroke Scale Certification (or must obtain by end of orientation) AND 8 hours of Stroke related Continuing Nursing Education (CNE) contact hours annually if assigned to: Critical Care, Intermediate Care Unit, Emergency Department, Neuro Units, Cardiology Inpatient at Hopewell, Peds ED, PACU, Interventional Radiology, CNI, Observation
Possesses excellent organizational, interpersonal, verbal and written communication skills. Ability to effectively manage multiple projects simultaneously and ability to respond quickly in a fast paced environment.
