Home Infusion Clinical Pharmacy Technician. The Clinical Pharmacy Technician will support clinical pharmacists and work closely with other members of the JHIS interdisciplinary team (nursing, pharmacy, dietitians, fulfillment and dispatch) to assure the needs of our patients and team are met in a safe, accurate and timely manner.
Job Description
- Communicating directly with patients, supporting essential tasks that result in safe and quality care for patients who receive intravenous medications or nutrition therapy at home
- Assure that patients have the supplies, DME, flush solution required to infuse their medications
- Facilitate timely medication and supply deliveries as well as retrieval of supplies from the patient’s home as needed
- Document all patient communications, triage responses, supply management or other activities in the patient medical record
- Coordinate/manage the schedule for patient deliveries, inventory and assist in the creation and management of supply templates in the pharmacy software system
- Entry of patient home medication profiles into the JHIS pharmacy software system
- Timely retrieval of patient lab test results from lab portals and assure transmission to prescriber practices
- Collaborate with team to assure JHIS has authorization to provide therapy, insurance coverage and active orders to provide prescribed therapy
- Participate in interdisciplinary committees and meetings
- The compounding of intravenous medications is not a responsibility of the clinical pharmacy technician
