Summary:
The HUP Comprehensive Sickle Cell Program is looking for a full-time social worker to join the team!
- Responsible for conducting bio-psychosocial assessments, providing support to patients and families, developing safe and timely discharge plans, and facilitating access to community resources and support services. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.
Responsibilities:
- Provide professional services to patients and families, including: assessment of psychosocial functioning, facilitating plans to address psychosocial needs, counseling for care planning and decision making, and community resource planning.
- Facilitate support groups.
- Work with patient and family to assure provision of choice and agency.
- Keep current with new providers, new technologies, treatment modalities and new resources using internet and other sources.
- Educate patient and family regarding scope and limitations of health care/insurance benefits.
- Assist patients in finding financial assistance for medications and supplies.
- Respect each patient’s personal dignity, right to privacy, and confidentiality.
- Collaborate with the health care team, as well as many disciplines, programs, department, and organizations in the accomplishment of these duties.
Education or Equivalent Experience:
- Education Specialization: Master's in Social Work (required).
- LSW preferred.
