We are seeking an experienced, knowledgeable Case Manager to work directly in our Emergency Department, collaborating closely with Physicians to determine that most appropriate level of care for a diverse array of patients and medical conditions.
Summary of Essential Duties:
- Provide case management services in collaboration with physicians, CSMC staff, and the Emergency Department
- Primary services include providing consultation regarding patient classification determination (inpatient or observation)
- Recommend alternatives to admission (non-acute resources, alternative levels of care)
- Work with the healthcare team to achieve optimal clinical, financial, and patient satisfaction outcomes
- Identifies and intervenes when plan variances occur
- Identifies practice patterns and quality issues that arise
- Initiates transition of care for patients who will be managed by the inpatient case managers on patient care units
Qualifications:
Education:
- Bachelor of Science, Nursing (BSN) required
- Master's Degree, Nursing (MSN) preferred
License/Certification:
- Current, unrestricted California RN License required
- BLS from the American Heart Association or American Red Cross required
- Certified Case Management RN preferred
Experience:
- A minimum of 5 years of case management experience required
- At least 3 of years of ED nursing experience preferred
- Emergency Department Case Manager experience preferred
- Experience using Interqual or Milliman required
- High level of competency performing medical necessity reviews with accepted criteria required
