Overview
As a School-Based Social Worker, you will provide essential mental health support and resource access to students, families, and staff, ensuring a nurturing and supportive school environment. By focusing on mental health, social determinants of health, and crisis intervention, you'll play a pivotal role in each student's journey to thrive academically and emotionally. Collaborate closely with a multidisciplinary team to deliver personalized care plans, therapeutic interventions, and community connections that elevate student health and resilience.
Responsibilities
- Provide compassionate social work services to students and families, addressing diverse needs related to health, finances, and community support.
- Develop and implement safe, timely treatment and discharge plans in partnership with students, families, and interdisciplinary team members.
- Employ evidence-based assessment tools and treatment approaches, using the latest clinical insights to inform interventions and support.
- Collaborate with a dedicated team of healthcare professionals, including primary care providers, psychiatrists, nurses, and more, to enhance overall quality of life.
- Assist with social determinants of health and refer students and families to appropriate resources to ensure a safe and supportive home and community environment.
- Spanish language skills required to effectively serve our diverse student population.
