Role Overview:
Demonstrating the tenets of our Professional Practice Model, Relationship Based Care® (RBC), provides professional nursing care at the advanced beginner level which includes assessment, planning, implementation and evaluation in accordance with care standards. Contributes to a safe and therapeutic environment that is centered on the needs of patients, caregivers and others showing concern for the patient's privacy, comfort, and emotional state. Treats patients with consideration of age, race, gender, gender identity, sexual orientation, religion, socioeconomic background, culture and disability in a caring, ethical and professional manner.
A CNI is expected to seek guidance and feedback from the preceptor and more advanced nurses and use organizational resources in caring for patients with increasing complexity.

Clinical Practice

ASSESSMENT: Performs an initial and ongoing assessment which encompasses the physical, psychosocial, cultural, spiritual, and educational needs of the patient and their caregiver(s). Determines and reports change in patient condition.
PLAN OF CARE: Using evidence and best practice standards, participates in the development of an individualized plan of care in collaboration with the patient, family and multidisciplinary team.
IMPLEMENTATION: Provides advanced beginner level of direct patient care and performs/assists with procedures. Adheres to policies, procedures and established standards of care in delivery of nursing services while creating a safe and therapeutic environment for patients, caregiver(s) and self.
EVALUATION: Evaluates progress toward expected outcomes and modifies individualized treatment plan.
PATIENT EDUCATION: Assesses patient and caregiver learning needs and provides education about the disease, treatment plan, actual and potential side effects of treatments and medications, and self care measures. Evaluates understanding of instructions provided.
PSYCHOSOCIAL SUPPORT: Provides psychosocial support to patients and caregivers while fostering a patient-centered environment.

Demonstrates understanding of unit based nurse sensitive indicators (CAUTI, CLABSI, HAPU, Falls, Extravasation, Patient satisfaction, Retained Foreign Objects, etc) and other quality indicators and associated data trends at the unit level.

Contributes to the learning environment by sharing knowledge and assisting with onboarding of new staff, as appropriate.
