The Montefiore Patient Care Coordinator maintains standards of the practice for nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and Nursing's Social Policy Statement.
Responsibilities
- The Patient Care Coordinator actualizes the vision, mission, values and balanced scorecard performance measures for Montefiore Medical Center.
- The Patient Care Coordinator participates in performance improvement/research activities; maintains professional practice standards and clinical expertise; and demonstrates leadership skills.
- The PCC demonstrates core and unit specific competencies based on the patient population served.
- The Patient Care Coordinator (PCC) contributes to the actualization of the vision, of the philosophy and the balanced performance measures of the discipline.
- He/she is committed to maintaining the standards of the practice of nursing and is responsible for the coordination, implementation, delivery and evaluation of the care to the neonate, pediatric, adolescent, adult and/or geriatric patient.
- The Patient Care Coordinator, in collaboration with and under the direction of the Administrative Nurse Manager participates in the coordination/implementation of fiscal, human and other resources needed to provide patient care services for a designated unit, location or patient population on a designated unit.
- Monitors performance of nursing personnel in maintaining MMC standards of practice of nursing.
- Coaches/provides feedback to nursing personnel as needed.
- Clinical title does not carry from unit to unit.
Requirements
- New York State License for Registered Professional Nurse and current registration or valid permit to practice in New York State as an R.N.
- Certification preferred in specialty or efforts to achieve certification.
- BSN Degree is required.
