The Bronx Health Collective (BHC), a mission driven and innovative Federally Qualified Health Center (FQHC) located in the South Bronx, is seeking a full-time Family Medicine or Internal Medicine Physician to join our dedicated team. We provide comprehensive care to children, adolescents, and adults in an underserved community. BHC is a program of Montefiore Medical Center and is affiliated with the Albert Einstein College of Medicine. We are deeply committed to delivering high-quality, compassionate, and comprehensive health care to those who need it most. Our beautifully designed, technologically advanced community health centers provide an enhanced medical home offering comprehensive primary care in Pediatrics, Reproductive Health, Adult Medicine, and HIV/AIDS care for underserved populations. Behavioral Health and Nutrition services are fully integrated and co-located within primary care, ensuring a holistic and coordinated approach to patient wellness. All of our health centers proudly hold and maintain NCQA's Patient-Centered Medical Home (PCMH) recognition. We also lead several special health initiatives, including programs in Child and Family Resiliency, Diabetes care, and Terra Firma-a unique medical-legal-mental health partnership serving unaccompanied immigrant children and families seeking humanitarian protection. Our multidisciplinary care teams include case managers, patient navigators, data analysts, and public health specialists who work collaboratively to support patient care and outcomes.
Responsibilities
- Current unrestricted NYS medical license and unrestricted DEA certificate, Infection Control and Basic Life Support certifications.
- Board Certified/Eligible in Family Medicine, Internal Medicine, or Medicine-Pediatrics (MD or DO).
- Experience and/or interest in a community-based setting. Excellent verbal and written communication skills.
- Comfortable with electronic medical records.
- Bilingual (English/Spanish) is a plus.
