Schedule: Day Shift - 20hr/week - Ideal candidate must be available within the hours 0500 -1730. Weekend rotation, every 6th weekend including 1 summer and 1 winter holiday requirement.

- Position Summary:
- Responsible for independently obtaining prior to admission medication and allergy history, completing immunization, Hepatitis C, and vaccine screening on all direct admissions, patients from the Emergency Department, and day surgery patients to assist the organization in maintaining patient and medication safety. As an essential pharmacy team member, obtaining accurate and precise information by interviewing the patient helps avoid medication duplications, exclusions, and errors. The information obtained is entered into the electronic health record and utilized by multifaceted healthcare professionals throughout the hospital encounter. Knowledge of pharmaceutical terminology and metric systems is necessary, in addition to excellent customer service skills and the ability to effectively work and communicate with patients, clinicians, pharmacists, and multidimensional care team providers within the community.
- Qualified individuals must have the ability with or without reasonable accommodation to perform the following duties:
- Review, collect, update and enter prior to admission medications on the patients current medication list.
- Complete computer entry of the home medications in a timely manner so information can be used as the foundation for the patient’s admission medication orders.
- Responsible for gathering information from various resources patient, family member, care giver, dispense history, physician orders, community pharmacies, mail order pharmacies, acute care facilities, etc. to ensure a complete and accurate entry into the hospital’s electronic health record system.
- Properly identify high-risk medications while alerting a Pharmacist and completing proper documentation in the hospital’s electronic record system I-Vent .
- Directly collaborates and communicates with the Pharmacist to resolve and issues related to the patient’s home medication list
- Review, collect, update and enter prior to admission allergies on the allergy list to include medications, environmental and food allergens and update proper contraindications, intolerances and side effects caused by allergens.
- Review, collect, update and enter immunizations includes: Tdap, COVID, FLU, PNU and order outstanding immunizations based off of the screening tool.
- Complete proper screening of substance use and order consultations based off of the screening tool.
- Complete proper screening of Hepatitis C and order lab testing basked off of the screening tool.
- Review, educate and document Meds on the Move.
- Review, educate and assist patients with MyChart Bedside enrollment.
- Review, educate and document ‘Taking Medications While in the Hospital’ education with patient.
- Complete proper discharge Medi-Set follow up.
- Effectively communicates and collaborates with multifaceted health care professionals to maintain appropriate patient and medication safety and to help identify patient specific needs language barriers, educational needs, financial needs, etc.
- Be familiar and comfortable working in high-risk environments BHE, ED, isolation rooms, etc.
- Have knowledge of isolation procedures and protocols annual N-95 Fit Testing is required .
- Based on professional licensure, may provide additional services within the scope of their licensure and or certification.
- The following duties are considered secondary to the primary duties listed above:
- Serve as a resource to provide onboarding training to new hires.
- Data retrieval, analysis and entry into the hospital’s electronic health record.
- Provide educational opportunities to the health system.
- Other duties as assigned.
Responsibilities:
- Minimum Required Qualifications:
- High school diploma or successful completion of General Educational Development GED exam is required.
- Must obtain 80% or higher on department administered exam on key elements of pharmaceutical knowledge and of the medication history intake process.
- General computer skills.
- Considerable knowledge of medication names both trade and generic , their uses, doses, and routes of administration.
- Must be able to identify medications based on markings.
- Excellent patient interview skills.
- Strong written and verbal communication skills with patients and other health care professionals.
- National pharmacy technician certification CPhT or other health care licensure LPN .
- Two 2 years of previous hospital health care or retail pharmacy practice experience.
- ATTENTION CONCENTRATION:
- The position requires the ability to attend to more than one aspect of a situation simultaneously. It is highly likely that multiple task demands are going to be required of the individual at the same time.
