ResponsibilitiesProvides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies.
Specifically, the Weight Management Primary Care Pharmacist will be a primary care pharmacist responsible for managing patients who fall out of a nurse protocol for weight management medications, primarily GLP-1 agonists. Responsibilities include Medication Therapy Management (MTM) or Medication Management Services (MMS) including comprehensive medication reviews, targeted medication reviews, and non-visit care.

QualificationsRequired Qualifications
- Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program OR Bachelor of Science (B.S.) AND a minimum of 2-years relevant experience.
Preferred Qualifications
- Residency training in ambulatory care.
- BCACP
- Experience providing care in an ambulatory care setting utilizing collaborative practice agreement.
- Completion of ASHP weight management certificate.
Additional Qualifications
- Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations preferred.
- Relevant applicable pharmacy experience working in a progressive and collaborative practice environment preferred.
- Experience and/or interest in teaching and/or precepting preferred.
- Commitment to excellence, accuracy, attention to detail and teamwork required.
- Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.
License or Certification
- Current pharmacist license by the Minnesota Board of Pharmacy, or eligible to obtain within a time frame compliant with the site’s applicable employee licensure policy (60-days).
- May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.
- Wisconsin pharmacist license preferred and required within 1 year of hire.
