The Staff Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.
The Staff Advanced Practice Nurse coordinates and administers psychiatric and medical care to all patients and residents of Carrier Clinic, and through a contract between Carrier and East Mountain Hospital, to all inpatients of East Mountain Hospital. Conforms to performance improvement standards of patient care as established through Policies and Procedures. Provides professional representation of HMH Carrier to other health professionals, community agencies, and the public.
4 hours a day Mon-Fri.
Minimum one year prescribing required.
Candidate should have a strong background in psych and substance treatment.
Manage managing consults in Blake Recovery and Carrier Inpatient Caseload.
Schedule: 4 hours per day- 9:15 AM - 12: 00 PM on Blake, attending morning meeting, seeing consults and follow-ups- 12:00 PM - 1:15 PM seeing inpatients on other units.
PMHNP with an emphasis on Addictions experience.
